The forms should be printed on A4 size paper from 01-03-2024 to 08-03-2024. Physical sale of forms will not be conducted.<\/li>\n<li>Duly filled APPLICATION FORMS along with supporting documents must be submitted either at the Directorate of Health Services, Lamphelpat, or at the Offices of CMOs in various locations (Jiribam, Churachandpur, Pherzawl, Tamenglong, Noney, Kangpokpi, Senapati, Ukhrul, Kamjong, Tengnoupal &amp; Chandel) on all working days from 04-03-2024 to 13-03-2024. A cash payment of Rs. 1000\/- (Rs. 500\/- for SC\/ST) is required as fees.<\/li>\n<li>Incomplete applications will be summarily rejected.
